Every patient completes a short registration and consent form before their consultation. It takes a few minutes, and it is the last thing standing between you and your appointment.

Why it matters
It records your consent
Your appointment happens by secure video rather than in a room, so we ask you to confirm you understand how the consultation works and how your information is handled before it starts.
It prepares your Dermatologist
The form captures your history, current medications and the details of your skin concern. Your Dermatologist reviews all of it before your call, so the appointment is spent on your skin rather than on paperwork.
Your consultation depends on it
A consultation cannot go ahead without a completed consent form. Finishing it early gives the coordinator team time to check everything over and come back to you if anything is missing.
When to complete it
As soon as you have booked. Your confirmation email arrives straight away with a link to the form, and the sooner it is done, the sooner your file is ready.
It must be completed before your appointment. If it is still outstanding on the day, your consultation may need to be rescheduled, so please do not leave it until the last moment.

Your photos are separate
Every consultation needs four clear photos of your skin condition, emailed to us at least one hour before your appointment. Your consultation can't go ahead without them.
- Photo 1: about 1 metre away
- Photo 2: about 20cm away
- Photo 3: about 5cm away
- Photo 4: as close as your camera allows
Email them to coordinator@teledermatologist.com.au with your full name in the subject line.
A nurse, carer or family member can help you take them, or call us and we'll arrange assistance.

Completing this form does not book an appointment on its own. If you have not booked a time with us yet, please do that first and the consent form link will come to you by email.
How your information is handled
The form is hosted in Google Forms and submits directly to the coordinator team. This website does not collect or store any of your health information. Our Privacy Policy sets out how your personal and health information is collected, stored and used, including consultation recordings and how your photographs are handled.
Book your consultation
Prepayment is required at booking. You'll need a referral for a Medicare rebate, but you can book without one.
